Milford Med Spa – Membership Terms & Conditions
This agreement outlines the terms of the Basic Beauty Membership. By enrolling, the member agrees to the following:
1. Membership Inclusions
The Basic Beauty Membership includes:
- Two (2) HydroFacials per year, redeemable once every six (6) months.
- $4 off per unit of Botox®, Dysport®, or Daxxify®.
- 10% off all services, excluding weight loss programs and Botox/Dysport/Daxxify neurotoxins.
- 10% off all retail skincare products.
- One (1) annual complimentary skin care consultation.
All benefits are valid only for the active member and may not be shared, gifted, or transferred.
2. Membership Fee
- Membership cost: $29.99 per month, billed automatically to the card on file.
- The member agrees to a one-year (12-month) commitment.
3. Benefit Redemption
- HydroFacials are issued semi-annually, one every six (6) months.
- Unused facials do not roll over beyond the membership year unless otherwise permitted by Milford Med Spa at its discretion.
- Discounts apply only while the membership is active and in good standing.
4. Freeze Policy
- Memberships may be frozen with approval.
- While frozen, no services or discounts may be redeemed, including HydroFacials.
- Monthly billing pauses during the freeze period.
- Membership benefits resume when the membership is unfrozen.
5. Auto-Renewal
The membership automatically renews at the end of the 12-month term unless cancelled as described in Section 6.
6. Cancellation Policy
- After the initial 12-month commitment, the member may cancel at any time.
- Cancellation must be submitted in writing, which may include:
- Text message
- Handwritten note delivered to Milford Med Spa
- All cancellation requests must be received before the next billing date to avoid charges for the upcoming cycle.
- Early cancellation before completing the 12-month commitment may result in fees or repayment of redeemed services at full price.
7. Payment Authorization
The member authorizes Milford Med Spa to charge the card on file for monthly dues and any outstanding balances. Failed payments may result in suspension or cancellation of membership benefits.
8. Non-Transferability
Membership benefits are for the enrolled member only and cannot be shared, transferred, or redeemed by anyone other than the member.
9. Terms Subject to Change
Milford Med Spa may update membership pricing, perks, or terms with 30 days’ written notice.
Enrollment in the Basic Beauty Membership signifies agreement to all terms listed above.
Advanced Glow Membership

This agreement outlines the terms of the Advanced Beauty Membership. By enrolling, the member agrees to the following:
1. Membership Inclusions
The Advanced Beauty Membership includes the following:
Included Benefits:
- Two (2) HydroFacials per year, redeemable one every six (6) months.
- One (1) SkinPen® Microneedling Treatment per year (value $350).
- $4 off per unit of Botox®, Dysport®, or Daxxify®.
- 10% off all services, excluding weight loss programs and Botox/Dysport/Daxxify neurotoxins.
- 10% off all retail skincare products.
- One (1) annual complimentary skin care consultation.
All benefits apply only to the active member and are not transferable.

- Membership cost: $37.99 per month, billed automatically to the card on file.
- The member agrees to a one-year (12-month) commitment.
3. Benefit Redemption

- While frozen, no services or discounts may be redeemed, including HydroFacials and the SkinPen® treatment.

6. Cancellation Policy
- After the initial 12-month commitment, members may cancel at any time.

- Handwritten note delivered to Milford Med Spa
- Requests must be received prior to the next billing date to avoid the following month’s charge.
- Early cancellation before completing the 12-month term may result in fees or repayment of redeemed services at full price.

The member authorizes Milford Med Spa to charge the card on file for monthly membership dues and any unpaid balances. Failed payments may lead to temporary suspension or cancellation of benefits.
8. Non-Transferability

Ultimate Glow Membership

This agreement outlines the terms of the Ultimate Beauty Membership. By enrolling, the member agrees to the following:
1. Membership Inclusions
The Ultimate Beauty Membership includes the following:
Included Benefits:
- Two (2) HydroFacials per year, redeemable one every six (6) months.
- One (1) SkinPen® microneedling treatment per year (value $350).
- One (1) syringe of dermal filler OR one (1) vial of Sculptra® annually.
- Member may choose their preferred option once annually.
- $4 off per unit of Botox®, Dysport®, or Daxxify®.
- 10% off all services, excluding weight loss programs and Botox/Dysport/Daxxify neurotoxins.
- 10% off all retail skincare products.
- One (1) annual complimentary skin care consultation.
All benefits apply only to the active member and are non-transferable.

- Unused services or injectable benefits do not roll over outside the membership year unless permitted at the discretion of Milford Med Spa.
- Discounts apply only while the membership is active and in good standing.

- During a freeze period, no services, injectables, or discounts may be redeemed.

By enrolling, the member agrees to the following:
1. Membership Inclusions
The GlowGetter 4X membership includes:
- Four (4) HydroFacials per year, redeemable once every three (3) months.
- $4 off per unit of Botox®, Dysport®, or Daxxify®.
- 10% off all services, excluding weight loss programs and Botox/Dysport/Daxxify neurotoxins.
- 10% off all retail skincare products.
- One (1) annual complimentary skin care consultation.
All benefits are valid only for the active member and are not transferable.
2. Membership Fee
- The cost of this membership is $46 per month, billed automatically to the card on file.
- The member agrees to a one-year (12-month) commitment.
3. Benefit Redemption
- HydroFacials are issued quarterly and may be redeemed once every three (3) months.
- Unused facials do not roll over beyond the membership year unless permitted by Milford Med Spa at its discretion.
- Discounts apply only while the membership is active and in good standing.
4. Freeze Policy
- Memberships may be frozen for personal, medical, or travel reasons upon request and approval.
- While frozen, no services or discounts may be redeemed, including quarterly HydroFacials.
- Monthly billing pauses during the freeze period.
- Membership benefits resume once the membership is unfrozen.
5. Auto-Renewal
- The membership automatically renews at the end of the 12-month term unless cancelled in accordance with Section 6.

- After the 12-month initial commitment is fulfilled, the member may cancel at any time.
